How To Unlock The Doctor
Complete True Moon Archon Quest
The Doctor's Trounce Domain becomes available once you've completed the True Moon Archon Quest. This will require playing through the latest Archon Quests added in the 6.3 Update (Luna IV).
6.3 Version Update GuideCan Be Fought Through The Adventurer's Handbook
The Doctor can be fought without having to do the Archon Quest through the Adventurer's Handbook. However, this will not reveal the location of the Trounce Domain on the map.

Collect Rewards Once A Week
As a World Boss, players can only collect the rewards for The Doctor once a week. You can fight it as much as you want, but once you collect the reward for the week, the subsequent fights will not yield any materials.

Pyro Is Not Recommended
The Doctor has high resistance against Pyro. It can still work, but you will be doing less damage compared to other elements.
Teams Can Be Pretty Flexible
Besides that, there's really no resitriction with the teams that you can bring to this fight. Having a healer or a shielder will make it easier since The Doctor deals a ton of damage.

How To Beat
Use Pillars To Escape Moonfall Impact
At the start of the fight and peridically during it, Dottore will fly into the air and unlease the Moonfall Impact. Use the pillars that will appear to escape from it.
If you get caught in Moonfall Impact, your active character will be one-shotted. Make sure to stay within the shadows of the pillar to stay protected from it.
Do Not Move Away When Told To Stay Still
When "Stay right where you are, or you'll be punished for crossing the line", do not move from where you are. If you do, the Doctor will freeze you, allowing his clones to attack you.
Turn Your Character's Back On Dottore When Instructed
When "Do not face The Doctor, for gazing upon a god invites punishment", turn your character away from Dottore. A red eye will appear above your character indicating this.
Keep you character's back to Dottore during this. Once the eye disappears, you can turn back to it.
He Can Invert Your Directional Controls
When "Affected by The Doctor's power, your sense of direction is now inverted", it simply switched your directional controls.
There's nothing you can really do here but wait for it to expire.
Head Into The Rift & Defeat The Clones
When the rifts appear, enter one and defeat the three Doctor clones there. This is recommended as defeating the clones will also deal damage to The Doctor.
You can also just exit the rift by locating the exit. This will bring you back to the battlefield.
