Nightmare-Bound in NTE is an Anomaly Commission (Location Type) that can be accepted inside the hospital west of the phone booth near Davidia Avenue. To unlock it, speak to the patient standing by the stairway.

What is Nightmare-Bound?

Escape Room-Like Content

Nightmare-Bound is a horror-themed Side Quest featuring escape room-style gameplay that can be played solo or with friends. As you search for clues across the hospital’s three floors to uncover the truth behind the incident, you must also evade an unkillable enemy stalking the halls. There are a total of four endings to uncover.

Nightmare-Bound / Nameless Hospital supports multiplayer. If you are not comfortable with horror content or tend to get lost easily, it is recommended to team up with other players before attempting this commission.

How to Play Multiplayer

Patrols Return You to Save Points

Savepoint

Unkillable enemies patrol the hospital and will chase you if they spot you nearby. If caught, you will be forcibly sent back to the savepoint on 1F, the counter on 2F, or in the corridor in front of the elevator on 3F, so proceed carefully.

TipsNote! There is no penalty for getting caught, so intentionally letting yourself be captured can be a useful way to return quickly if you get lost or want to shorten travel time.

Boom Boom Mist Stuns Patrols

Boom Boom Mist

When chased by a patrolling enemy, using "Boom Boom Mist" will stun them and create an opportunity to escape. You can obtain one on 1F, two on 2F, and three on 3F.

Any Ending Gives Staff Elevator Card

Staff Elevator Card

Clearing any one of the endings completes the commission and grants the rewards. At that point, you receive the Staff Elevator Card, which lets you freely use the elevator inside the Nameless Hospital.

TipsTip! Endings 3 and 4 require access to the elevator, so clear Ending 1 or 2 first to obtain the Staff Elevator Card.

How to Restore Power to 1F

From the 1F Reception Area to the Power Room.

The power room can be accessed from the 1F reception area. Inside are several breaker panels, and activating the four inactive breakers in order from back to front and left to right will restore power to 1F, allowing you to toggle the room and corridor lights ON or OFF.

TipsTip! Right after the power is restored, an unkillable enemy will appear outside the door, dash through it as shown in the video above to avoid getting caught.

2F Floods by Chance

Reception 2F Note Flood

2F has both flooded and non-flooded variants, and the version appears to be randomly determined when entering the Anomalous Realm. If the document on the 2F reception counter mentions a flooding phenomenon, the flooded variant will appear.

TipsTips! Once you move away from the 2F Reception Counter after reading the note, an enemy will appear, and the floor will become flooded.

Recover HP with Emergency Med Kits

How to recover HP

Using an Emergency Medical Kit found on walls or desks restores a fixed amount of HP. Use them whenever you have taken significant damage during combat.

Get 50,000 Fons in Three Locations

From the 1F starting point, head down the left corridor, turn right at the dead end, continue straight, then take the right path at the fork. The item can be obtained from the safe inside the room on the left.
*Requires Keychain: 1F and Defeating 3 Enemies
Safe in further room
Obtained from the corridor beside the locked door leading to the 2F stairwell.
*Requires a long detour through the central ward
Hole on Ceiling
In the 2F room with a hole in the ceiling, use your Esper Ability to reach 3F and obtain it from the safe.
*Requires Keychain: 2F

You can obtain a total of 150,000 Fons from safes inside the Anomalous Realm, so make sure to collect them along the way.

Should I Clear All Endings?

Should I clear

Each ending rewards you with Annulith and Fons, so it is recommended to clear all of them to avoid missing any rewards. However, if the horror elements feel too stressful or multiplayer becomes difficult to manage, there is no need to force yourself to continue.

What if the Floor Doesn’t Flood?

No Flood

If the note in the 2F reception area matches the one shown above, the floor will not become flooded.

The mechanic in finding the Central Ward Key slightly changes based on the flood or non-flooded floor variant. If the note in 2F reception mentions the lights instead of flood, you will need to switch off the light in the Central Ward Key room to find it.

Can't Get the 2F Central Ward Key?

Flooded Floor Variant

Water Level Increase

To obtain the Central Ward Key in the Flooded Floor variant, you must raise the water level further by turning on the faucet in the 2F Bathroom. Make sure to stop by the bathroom and activate the faucet before heading to the room containing the Central Ward Key.

Lights (Non-Flooded) Variant

Lights Variant

To obtain the Central Ward Key in the Lights (Non-Flooded) variant, enter the locked Central Ward Key room through the hole-in-the-wall room and then turn on the lights.

TipsTip!

In the Lights Variant, toggling the lights on and off will reveal hidden keys in 2F. If you cannot find the key in its designated room, try switching the lights on or off.

What If I Can't go to 3F?

Staff Elevator Key

To access 3F, you must clear one ending to obtain the Staff Elevator Card and use the elevator. Start by completing Ending 1 or 2, as both can be cleared using only 1F and 2F, to unlock the Staff Elevator Card.

Clues Required for Each Ending

3 Clues

Ending 1 requires the "Anomaly Disorder" clues, Ending 2 requires the Surveillance Record clues, and Ending 3 requires the Anomaly Experiment clues. Ending 4 requires Anomaly Experiment Clues III and VI, along with V???'s Notes.

TipsNote!

The clue numbers required for Endings 1, 2, and 3 are not fixed, and collecting at least 3 clues is enough to clear them.

Where Are the Ending 4 Clues?

For Ending 4, all required clues are located on 3F, and hidden rooms and passages must be revealed by combining "Jigsaw" items. Since Ending 4 involves many steps and is highly complex, it is recommended to refer to the walkthrough chart video.

Can't Put Mysty in Container?

Mysty may be about to disappear, or you may be using the wrong container. Once guided out, Mysty will disappear after a certain amount of time, so if you take too long, return and guide Mysty out again. It is also important to reach the container that transforms Mysty into a Jigsaw piece quickly.

How to Solve Light Switch Puzzle?

The light switch puzzle for Ending 4 can be solved with a sequence of four toggles: 2 Short → Long → Long → Long → Long → 2 Short . During the "Long" pause with the lights on, wait before the buzzing sound ends before switching off.

TipsNote!

Toggling the switch on and off four additional times will reset it to its initial state, so if the bookshelf does not move, adjust your timing and try again.

Restart If Caught by an Enemy?

Savepoint

The savepoints on 2F and 3F are marked by floppy disks.

If an enemy catches you, you will be sent back to a designated location on the current floor, but obtained items and completed puzzles will remain unaffected. However, repeatedly getting caught will prolong the run and extend the horror experience.

From 1F Start to Anomaly Disorder Clue I

From the 1F starting point, head to the counter on the front-right side. Investigate the yellow computer to obtain "Anomaly Disorder Clue I".

Obtain Keychain: 1F

From obtaining Anomaly Disorder Clue I to Keychain: 1F

After obtaining Anomaly Disorder Clue I, take the left corridor, turn right at the end, then continue straight and turn right at the fork. Head straight to the large reception hall and approach the counter on the right. Investigate the counter to obtain "Keychain: 1F".

Obtain Anomaly Disorder Clue II

From obtaining Keychain: 1F to Anomaly Disorder Clue II.

After obtaining Keychain: 1F from the reception table, head to the far-left side of the waiting hall and enter the room at the leftmost end. Investigate the yellow computer to obtain "Anomaly Disorder Clue II".

Obtain Anomaly Disorder Clue III

From obtaining Anomaly Disorder Clue II to Anomaly Disorder Clue III.

Leave the room where you obtained Anomaly Disorder Clue II, return to the reception hall, and head straight into the corridor. Continue straight at the fork, then enter the room on the right. Investigate the yellow computer to obtain "Anomaly Disorder Clue III".

Obtain Spare Key: 1F

From obtaining Anomaly Disorder Clue III to Spare Key: 1F.

Go through the door in the room where you obtained Anomaly Disorder Clue III and move to the room two doors ahead. Exit into the left corridor, continue straight, and enter the room directly ahead. Open the door on the left with green light and proceed through the next three rooms. Investigate the Storage Cabinet to obtain "Spare Key: 1F".

Take the Stairs to Floor 2

From obtaining Spare Key: 1F to Floor 2.

Destroy the trash beside the Storage Cabinet where you obtained Spare Key: 1F using an attack, then open the door and exit into the corridor. Open the door to the front-left and enter the stairwell. Take the stairs to Floor 2.

View Clues on the 3 Projectors

From arriving at Floor 2 via stairs to Projectors.

After reaching Floor 2 and entering the corridor, head into the reception hall on the right and enter the room behind the counter on the left. Insert Anomaly Disorder Clue I, II, and III into the projectors to play the footage and clear Ending 1.

Obtain Surveillance Record Clue I

From the 1F starting point to Surveillance Record Clue I.

From the 1F starting point, head down the left corridor and turn right at the end. Continue straight and enter the room on the right just before the fork. Weave past or jump over the obstacles inside and proceed to the back of the room, where investigating the documents obtains "Surveillance Record Clue I".

Take the Elevator to 2F

From obtaining Surveillance Record Clue I to Elevator.

Return to the corridor through the same door you used to enter the room where you obtained Surveillance Record Clue I, then take the elevator on the front-left side to Floor 2. The elevator becomes available after clearing Ending 1.

TipsTip! If you have not cleared Ending 1 yet, you can still access 2F by following Steps 6-7 of the Ending 1 walkthrough and using the stairs.

Obtain Surveillance Record Clue II

From the 2F elevator to Surveillance Record Clue II.

Exit the elevator and head right to reach a large reception area. With the counter behind you, take the far-left corridor, turn left at the end, and enter the room on the right past the bathroom. Investigating the documents on the desk obtains "Surveillance Record Clue II".

TipsTip! Since the route ends back at the reception area, it is recommended to defeat the enemies there beforehand.
Obtain Surveillance Record Clue III

From obtaining Surveillance Record Clue II to Surveillance Record Clue III.

Leave the room where you obtained Surveillance Record Clue II, return to the corridor, head slightly right, and enter the room on the right. Investigate the corkboard to obtain "Surveillance Record Clue III".

View the Clues on the 3 Projectors

From obtaining Surveillance Record Clue III to Projectors.

Leave the room where you obtained Surveillance Record Clue III, return to the corridor, head left, turn right at the end, and continue straight back to the reception area. Cross to the right side and enter the room behind the counter. Insert Surveillance Record Clues I, II, and III into the projectors to play the footage and clear Ending 2.

First, take the elevator on Floor 1 to Floor 3. Elevator access is required for Ending 3, so clear Ending 1 or 2 beforehand to obtain the "Staff Elevator Card".

Get Key - North District

From the 3F elevator to Key - North District.

Exit the elevator, proceed a short distance down the corridor, and approach the container on the floor to your left. Investigate the container for a dialogue to obtain Key - North District.

Get Anomaly Experiment Clue II

From obtaining Key - North District to Anomaly Experiment Clue II.

At the end of the corridor in front of the elevator, open the door on the left to enter a room. Proceed straight through the room, exit through the door on the right into the corridor, and enter the room on the right. Investigate the documents on the counter to obtain "Anomaly Experiment Clue II".

Get Key - South District

From obtaining Anomaly Experiment Clue II to Key - South District.

Leave the room where you obtained Anomaly Experiment Clue II, return to the corridor, and head back into the room on the left. Move to the left side of the room to return to the corridor in front of the elevator, then continue straight and approach the clock on the left wall. Investigate the clock to obtain "Key - South District".

Get Keychain - Office

From obtaining Key - South District to Keychain - Office.

Facing the clock where you obtained Key - South District, open the door on the right and proceed forward. Take the right path at the fork ahead, then enter the room at the dead end. Open the left side of the shelf in front of you to obtain "Keychain - Office".

TipsTip! If you remain too long in the room containing Keychain - Office, you will be forcibly sent back to the room entrance.
Get Anomaly Experiment Clue IV

From obtaining Keychain - Office to Anomaly Experiment Clue IV.

Leave the room where you obtained Keychain - Office, return to the corridor, and open the door directly ahead. Proceed a short distance down the next corridor to the right, then enter the room on the right. Move to the left side of the room and investigate the documents on the desk to obtain "Anomaly Experiment Clue IV".

TipsTip! If you stay too long in the room containing Anomaly Experiment Clue IV, you will be forcibly sent back outside the room.
Get Anomaly Experiment Clue V

From obtaining Anomaly Experiment Clue IV to Anomaly Experiment Clue V.

Facing the desk where you obtained Anomaly Experiment Clue IV, head left and open the door into the next room. Open the door at the back-left side of the room to enter the corridor, then open the door directly ahead to enter a large room. Investigate the documents on the bed to obtain "Anomaly Experiment Clue V".

TipsTip! Enemies will appear upon entering the room, so defeat them all. Additionally, remaining in the room for too long will forcibly eject you from the room.
Take the Elevator to Floor 2

From obtaining Anomaly Experiment Clue V to Elevator.

After obtaining Anomaly Experiment Clue V, return to the corridor through the same door you entered. Head right, turn right at the dead end, then go through the second door on the left to return to the fork. Take the right path at the fork, turn left just before the door at the dead end, and take the elevator down to Floor 2.

View the Clues on the 3 Projectors

From arriving on Floor 2 via elevator to Projectors.

On the 2nd floor, get off the elevator and head right, then enter the room behind the counter in the reception area. Insert Anomaly Experiment Clues II, IV, and V into the projectors, and a video will play to clear Ending 3.

Ending 4: "Voice of the Nightmare"

Ending 4 is complex, so following the video guide is recommended.

Speak with the patient facing the wall near the stairs. From the dialogue options, choose "(Close eyes)" and then "Enter" to enter Nameless Hospital.

Take the Elevator to 2F

From the 1F starting point to Elevator.

First, take the 1F elevator up to 2F. Elevator travel is required to clear Ending 4, so clear Ending 1 or 2 beforehand to obtain the Staff Elevator Card.

Obtain Keychain: 2F

From the 2F elevator to Keychain: 2F.

With your back to the counter in the 2F waiting area, take the back-left corridor, turn left at the dead end, and enter the room on the right past the bathroom. Investigate the corkboard on the wall to obtain "Keychain: 2F".

Turn off the lights if 2F is not flooded

If the area is flooded, you can obtain the item directly from the corkboard. However, if it is not flooded, the corkboard will be invisible, so turn off the room lights to make it appear.

Enter Room with a Hole in the Wall

From obtaining Keychain: 2F to room with wall hole.

Exit the room where you got Keychain: 2F, head left down the corridor, turn right at the dead end, take the right path at the next fork, and open the second door on the right to enter the room. There is a hole in the wall, and you can pass through it to reach the next room.

Get "Jigsaw" - Type IV via Ceiling Hole

From room with a wall hole to "Jigsaw" - Type IV.

Move to the next room through the wall hole, and climb up to 3F via the ceiling hole. Investigate the container on the shelf to obtain "Jigsaw" - Type IV.

How to Reach 3F Using Zero

If you are using the Main Character, you can climb onto the locker in the room, activate the Skill, then switch characters mid-air and jump. Using Nanally to ignore gravity and climb is easier, but if you do not have her, you can still attempt it with the Main Character.

Take the Elevator to 3F

From obtaining "Jigsaw" - Type IV to going up to 3F.

Return to the same elevator you used to get to 2F, and take it up to 3F. When leaving the room with the ceiling hole, you can attack and destroy the trash blocking the door to make it passable.

Obtain Key - North District

From the 3F elevator to Key - North District.

Exit the elevator, proceed a short distance down the corridor, and approach the container on the floor to your left. Investigate the container for a dialogue to obtain Key - North District.

Assemble "Jigsaw" - Type IV

From obtaining Key - North District to Hidden Room.

At the dead end of the corridor in front of the elevator, open the door on the left and enter the room. You can assemble jigsaws on the table in the center of the room. First, assemble "Jigsaw" - Type IV to reveal a hidden room.

Obtain V???'s Notes

From revealing the Hidden Room to V???'s Notes.

Enter the hidden room revealed by "Jigsaw" - Type IV, and approach the wall on the right. As you get close, the wall vanishes and a hidden space appears. Next, investigate the documents on the desk four times to obtain V???'s Notes.

Obtain "Jigsaw" - Type I

From obtaining V???'s Notes to "Jigsaw" - Type I.

Return from the hidden room to the room where you assembled the jigsaw, open the door directly in front of you to step out into the corridor, and open the door on the right to enter the room. Investigate the container on top of the counter to obtain "Jigsaw" - Type I.

Assemble "Jigsaw" - Type I

From "Jigsaw" - Type I to opening a new corridor.

Exit the room where you got "Jigsaw" - Type I, return to the corridor, head left, and go back to the room where you assemble jigsaws. Approach the central table and assemble "Jigsaw" - Type I to open a new corridor.

Defeat the 4 Enemies
Defeat the 4 Enemies

When you assemble "Jigsaw" - Type I, four enemies will appear in the room. Since you will need to assemble more Jigsaws later in the route, defeat them all now.

Obtain Anomaly Experiment Clue III

From 4 enemies to Anomaly Experiment Clue III.

Head straight down the newly opened corridor, turn right at the dead end, and enter the room through the door on the right. Investigate the documents on the desk to obtain Anomaly Experiment Clue III.

Can Also Take a Detour

An invincible enemy patrols the newly opened corridor and may occasionally block the direct route. If this happens, take the right path at the fork, then turn left at each dead end to move counterclockwise through the corridor and reach the target room.

Bring Mysty Out

From obtaining Anomaly Experiment Clue III to Mysty.

Return from the room where you got Anomaly Experiment Clue III to the corridor, and open the door directly in front of you to enter the room. Mysty is in the rightmost container among those lined up on the counter, so approach and select "Come with me".

TipsTip! Mysty will disappear after a certain amount of time, but they can be taken out from the original container again.
Turn Mysty into "Jigsaw" - Type II

From retrieving Mysty to "Jigsaw" - Type II.

Return from Mysty’s room to the corridor, head right, then turn left at the dead end. At the next fork, take the left path and open the door on the right to enter the room. Place Mysty in the leftmost container lined up on the counter to transform it into a Jigsaw, then investigate the container to obtain "Jigsaw" - Type II.

TipsTip! An invincible enemy patrols the area, but the target room can also be reached by taking the corridor from the opposite direction above.
Assemble "Jigsaw" - Type II

From "Jigsaw" - Type II to 2nd Hidden Room Reveal.

Exit the room where you got "Jigsaw" - Type II, return to the corridor, head left, turn left at the dead end, then go straight back to the room where you assemble jigsaws. Approach the central table and assemble "Jigsaw" - Type II to reveal a second hidden room.

Move to the 2nd Hidden Room

From revealing the 2nd Hidden Room to reaching it.

From the room where you assembled the Jigsaw, return to the corridor patrolled by the invincible enemy. Take the right path at the fork, turn left at the dead end, and open the door at the end to enter the second hidden room. Use the spot directly across from the vending-machine-shaped enemy as a landmark.

TipsTip! An invincible enemy patrols the area, but the target room can also be reached by taking the corridor from the opposite direction above.
Solve the Switch Puzzle

From entering the 2nd Hidden Room to Switch Puzzle.

Solve the switch puzzle between the bookshelves in the second hidden room. Toggle the switch in this order: Short → Short → Long → Long → Long → Long → Short → Short, and the adjacent bookshelf will rotate, opening the path to the 3rd Hidden Room.

Obtain Anomaly Experiment Clue VI

From Puzzle Clear to Anomaly Experiment Clue VI.

Pass alongside the rotated bookshelf into the third hidden room, and approach the yellow PC on top of the shelf at the back, facing the entrance. Investigate the yellow PC to obtain Anomaly Experiment Clue VI.

Return to the Elevator

From obtaining Anomaly Experiment Clue VI to Elevator.

Leave the 3rd hidden room and return to the 2nd hidden room. From there, go through the corridor back to the room where you assembled the Jigsaws. Return to the corridor in front of the elevator, then take the elevator down to 2F.

View Clues with the 3 Projectors

From arriving on 2F by elevator to Projectors.

Step off the elevator on 2F, head right, and enter the room behind the counter in the reception area. Insert V???'s Notes, Anomaly Experiment Clue III, and Anomaly Experiment Clue VI into the projectors, the footage plays, and Ending 4 is complete.
