The Fundoshi can be acquired by visiting all Hot Springs in Tsushima. You need to unlock all the 3 islands via the main story in order to do this.

Hot Springs Increase Health
Dipping into hot springs not only revitalizes Jin's health but also increases his maximum health. You can also hear Jin's thoughts about certain events and characters while diving into these comforting oases.

No Noise Stealth
The Fundoshi is a very effective armor if you want to breeze through enemy guards and sneak into fortresses and camps. It negates the noise generated while running and sprinting, giving you a much better chance to quickly assassinate enemy troops and leaders.
No Other Benefits
Aside from keeping you quiet, the Fundoshi offers no other combat benefits. Wearing it and fighting heavily armored enemies looks out of place but it has its charms.
