The Great Omenkiller Cleaver can be obtained by defeating the Omenkiller in the Perfumer's Grotto in the Altus Plateau region. If you've activated the site of grace "Outer Wall Phantom Tree" then you should be close.

Waypoint To The Altus Plateau
The Altus Plateau can be accessed by using the Grand Lift Of Dectus, which can be located on the northernmost part of the Liurnia Of The Lakes region. To use the life, you'll need both the left & right Dectus medallion.
