Upon examining the Blue Request Note, open your map and you will see the location of 4 Medallions. Use this map and destroy each Blue Medallions.

It Is Okay To Destroy The Medallion Before Examining The Request Notes
The Blue Medallions already exist even before looking at the map. You can opt to destroy them once found then check the Blue Request Note after.
It Is Recommended To Explore Before Climbing The 2nd Floor Of The Building
Once you climbed the second floor and obtained a gear, a powerful hammer-weilding enemy will appear circling around the area. You will have no time to search for the medallions so make sure to explore before climbing the ladder.
1st Blue Medallion Location
The first medallion can be found at the window at the second floor of a building. You can shoot the medallion itself so carefully aim for it to destroy.
2nd Blue Medallion Location
The second medallion can be found pasted in the corner of the building. Check the map for the location and head to the area.
3rd Blue Medallion Location
The third medallion can be found inside the cow shed. It is difficult to destroy inside the shed so head outside and shoot it from the window.
4th Blue Medallion Location
The fourth medallion can be found on the beams of the building. It is difficult to spot once you are inside the building so instead search for it right before entering the building.
5th Blue Medallion Location
The 5th Medallion can be a bit difficult to find. From the Blue Request Note, head left until you reach a wooden fence. While there is a fence on your right, use the handgun to destroy the medallion.
After Finishing, Report At The Weapon Merchant
After destroying 5 Blue Medallions, you will not receive Spinels unless you report it. Report it at the Weapon Merchant encountered at the early part of Chapter 2 to complete the request.
