To unlock Duchess in Elden Ring Nightreign, you must first obtain Old Pocketwatch. After obtaining the item, give the item to the hooded girl to unlock Dutches.

To unlock Duchess in Nightreign, you first need to obtain the Old Pocketwatch. You can get this on the first day even after losing.
Give The Old Pocketwatch To The Hooded Girl

Once you obtain the Old Pocketwatch, show it to the hooded girl at the Roundtable Hold. The Old Pocketwatch can be equipped as a Relic, and it won't disappear even after showing it to the girl, so you can still equip it as a Relic.
Duchess Will Be Unlocked

Upon showing the Old Pocketwatch to the hooded girl at the Roundtable Hold, the Duchess will finally be unlocked, and you will be able to use Duchess.
Duchess Is Important To Unlock Revenant

Her Passive Makes Her Invisible
The Duchess can use her Ultimate Art to make herself and nearby allies invisible, preventing detection by enemies, including bosses. While invisible, even if you attack stealthily, enemies will not notice you. It will allow you to aim for Critical Strikes against mobs and continuous damage against tough enemies. It's also perfect for when allies are in low HP, you can regroup using this skill.
