There are specific missions in the game that will bar you from fast travelling or even leaving the mission objective area. You can only opt out of it by abandoning the Mission. This will restart your mission progress.
Character Sex & Model Design Permanent
In the Story Mission: Gear Up, you will be given the chance to customize how your character looks like. Take time to go through the selection since the character's sex & model design cannot be changed later on in the game.

Throughout the story, you'll be faced with choices that could have an effect on the ending you'll get. You can't go back to these specific parts unless you create a separate save file or start a new game.
Choice 1: Kill or Leave Mickey Behind
The first choice you will run into is whether Mickey will live or die. Killing her will leave her body free for looting, while letting her live will have her going in a redemption journey. She'll also leave behind her iconic helmet behind for you.

In the cutscene after killing Ethan, Joseph will ask you to kill him. You can choose to do so or deny him his request by not doing anything. It doesn't really affect anything as the only different is he'll have additional dialogue.
