In my game I have been there twice and there is no alter to solved the puzzle and raised the ladder and jumped across there is no alter to push
so it will not register

You can't access this tomb the first time you p*** it, since you won't have the CLIMBING AXES. Later, after Lara reunites with Jonah and heads for Kuwak Yaku, you will p*** a side path that leads back to the starting area.
